There is no official form. A few sentences covering these points is plenty, and it saves us writing back to ask.

You don’t have to be asking for something.
Volunteers keep Ellie’s Garden going. Libraries and schools tell us where the need is. Readers tell us which of her books still turn up in classrooms.
If you knew Ellie, or were taught by her, or simply read one of her books at the right age — we would genuinely like to hear it.
